Hello, whenever i try to upload the following list of contacts using the following list using the template provided for bulk field upload, please let me know if there's something I'm doing wrong or if someone else has had this issue.

That typically happens when you replicate a tag. You can only use a tag once, and that one is probably associated to some other field value. You'll need to upload the tags as well, creating a unique tag for that value.
