Customize the inquiry form (request form) of the help center

I want to customize the inquiry form (request form) of the help center and insert a completion screen after the input screen. How can I make it possible? Is it possible to realize?

As a premise, I want to use the help page TOP that appears after input screen as help page. (It doesn't mean that help page TOP is regarded as a completion screen.)

    You should be able to set this up using our Theming Center, but sadly, we cannot provide instructions or advise on custom code.

    In Support, we’re happy to help demonstrate, troubleshoot, and ensure functionality of our default code. We’d be able to work through an issue or question with a specific part of our default code, but unfortunately we cannot build custom code for your Guide or help you troubleshoot your customized code.

    We do have the following resources available to help your developers regarding Guide customization:
