Custom Ticket Form Not Displaying on Branded Help Center Page

I'm facing an issue and I'm hoping someone here might have encountered something similar or could guide me in the right direction.

Issue: Even after creating and enabling a custom ticket form in my Zendesk account, when I go to the requests/new page on my branded Help Center, I'm only seeing a generic form with a title and description. My custom fields aren’t displaying.

Steps I've Taken:

  1. Created a custom ticket form with specific fields tailored to our requirements.
  2. Made sure the form is enabled.
  3. Set the form to be visible specifically for my brand.
  4. Checked the requests/new page, but the changes aren’t reflecting.

I've double-checked all the settings, but I can't seem to pinpoint where I'm going wrong. Is there a step I’m missing or a common mistake that might be preventing the custom form from appearing? Has anyone else faced this issue?
