Would anybody know where we can edit our custom form? Ive checked in guide editor but the HTML isn't what shows on the form. Is there anywhere else i can check, i feel like i searched everything and have come up empty handed. Thank you

Are you attempting to edit the fields and descriptions? If so, that's managed in the Support UI under ticket forms. More info here: Managing ticket forms. Depending on your role permissions, you may need to contact an admin on the account to assist with any changes. Both the form and the fields have options to add separate customer facing names.
If you're looking to modify the look of the form, that's managed either by directly editing the HTML or using a new theme, specifically the new requests page that you show in your second screenshot. More information here: Customizing your help center theme
