Is there a way to send a request to an end users manager for approval within Zendesk ?
We have mapped each user so that it also imports the user manager from AD in form of an email address.
Is it possible to map that to a field within a form and then use a macro / automation to email the request to ask for approval

I think there's a way to do this if the field (manager email) is saved to the Organization, but I'm not sure how to do it via user fields.... The other idea, as @brett13 mentioned, there are apps you might want to try.
Where does that email address live? Is it a user field? If so, you may be able to do something with a trigger. Here are the references for user fields:
